Lake County (PLE) Peers With Lived Experience & Expertise
Who Are We?
We are a collective of community members working with members of the LWLC Network to support the goals and objectives of the (CHIP) Community Health Improvement Plan.
What Is Our Purpose?
The Peers with Lived Experience & Expertise Working Group operates with the purpose to build and sustain health equity-focused working relationships between community members, community-serving organizations (CSOs), community-based organizations (CBOs) and the Lake County Health Department.
Our intention and all related activity, based on the structure built by the CDC Foundation and collaborative-based collective impact, guides all parties along a spectrum of community engagement to community ownership through the CHIP (Community Health Improvement Plan).
We measurably provide the community* with opportunities to:
- Examine relevant information
- Assist in gathering input
- Ensure community needs and assets are integrated into process and inform planning
- Ensure and grow community capacity to play leadership roles in the implementation of decisions
- Foster democratic participation and equity by bridging the divide between community and governance through community-driven decision making.
